Community Nursing Services – Neurocare Services Pathway


Neurocare Services

The Neurological Council of WA (NCWA) provides a free community neurological nursing service called Neurocare to support all people living with the impact of a neurological condition. This service can be accessed whilst the patient is waiting to get a diagnosis from a neurologist.


Neurocare community nurses provide in-house visits and can assist with:

  • information about the course of the patient’s condition
  • education about treatment and drugs
  • liaise with GPs, neurologists, allied health on behalf of the patient
  • provide resources and referrals for personal and peer support
  • provide support to enable the person to function in their home and community
  • track progression of condition, symptoms and support


Neurocare will provide community neurological nursing services at:

  • pre-diagnosis
  • diagnosis
  • post diagnosis


Services have a focus on equity and accessibility and are provided according to clients’ need, not their diagnosis. Management of the impact on everyday life is not diagnosis specific. The service can play a role whilst waiting to access neurologist.


Request Neurocare Services

Check eligibility:

  • patient with diagnosed neurological condition
  • patient with suspected/undiagnosed neurological symptoms
  • all neurological conditions and ages accepted
  • locations: integrated service across Perth Metropolitan, Great Southern, South West, Mid-West and Wheatbelt regions


Discuss with the patient:

  • they will be assessed for support with symptom management and provided with information and tools to best maximise their wellbeing
  • no fee is payable for services
  • wait times – aim to see patients within 10 work days, triage for urgent referrals
  • visits are in office hours and can be at patient’s home, local hospital or GP practice
